Appointment
Office visits are scheduled by appointment only. We will try our best to accommodate your urgent request. When you call for an appointment, please tell our staff the reason of your visit, contact information, referral, and insurance coverage.
Things to Bring to Your Appointment: Completed medical forms; list of current medications including dosage; description of any allergic reaction; your insurance card, referral and prior authorization if required by your insurance; test results and CT/MRI film or CD if any; your co-pay or payment.
In order to make the most of your visit, please be prepared to provide a detailed medical history. Please let us know of your recent hospitalization, change of medication, and tests if any.
Please provide records from your other physicians and tell us what tests they have already scheduled for you so we can avoid duplication.

Insurance and Fees
We accept major insurance plans and will assist you in filing claims with your insurance carrier. Please contact our office or your insurance company for any questions in regards to insurance plans that the clinic accepts.
Currently, we accept the following (and many other) insurance plans: Medicare, Anthem BCBS, TriCare, Aetna, Cigna, United Healthcare, PMG and SCCIPA. Please call our office to verify insurance eligibility.
Self pay and insurance co-pays are collect at the time of service.
Fees for office visits are determined by the complexity of the problem as defined by diagnosis, research, and coordination of treatment with other physicians. We do all we can to keep the cost of health care down, without compromising the quality of care you receive. The patient is, however, ultimately responsible for the bill.
Prescriptions
A new prescription usually requires an office visit. It is most convenient for you to request refills 1 or 2 weeks in advance by calling your pharmacy and asking them to contact us.
Please call before you run out of medicine, as you may be requested to schedule an appointment in our office if it has been too long since the original prescription was provided.
Please make sure to plan ahead for weekends or travel needs.
It may be difficult or impossible to renew a prescription during weekends or after hours when the doctor on-call does not have your medical records. Calling our office rather than the pharmacy usually delays the refill of a prescription.
If you choose to call our office, please call during normal business hours and provide the following:
· Patient’s name, date-of-birth, social security number, address, and phone number
· Name, strength, and dosing instructions of the medications you need
· Phone & fax number of your pharmacy
Calling our Clinic
Please call and let us know as soon as possible if you need to change or cancel an appointment so that we can serve another patient during that time.
Every attempt will be made to respond to phone calls on the same day; however, the vagaries of medical practice (emergencies, scheduled patients, and triage) do not always permit that.
If your need is urgent, please let our staff know. If Dr. Wang is not able to adequately answer a question or request by phone, then an office visit may be suggested. Sometimes the best care requires an appointment with the physician.
For a life threatening emergency, please call 911 and go to the Emergency Room. Your ER physician will contact us if needed.
The after hour coverage is for urgent matters only. Please do not use that for regular prescription refills.
